Mental Health Services

COTA Health provides a range of services for adults 16 years of age and older who are experiencing mental health challenges.

Scarborough Step By Step program

 

The Scarborough Step By Step Supportive Housing Program provides rent-subsidized accommodation and support to people living with mental health challenges in the Scarborough community. Each tenant occupies his/her our apartment or housing unit.

Mental Health Case Management

 

The Mental Health Case Management service provides individualized support to clients living with a serious mental illness in the City of Toronto (inclusive of Etobicoke, Scarborough, North York and downtown Toronto).

Hostel Outreach Program

 

The Hostel Outreach Program (HOP) provides individualized support to men who are homeless or under-housed and are living with a serious mental illness.

Geriatric Mental Health Case Management

 

COTA Health’s Geriatric Mental Health (GMH) Case Management service provides individualized support to clients living with age-related dementia, and clients over age 65 living with a serious mental illness. This service is available in Etobicoke, North York, Scarborough and downtown Toronto.

Boarding Home Site Support

 

The Boarding Home Site Support program assigns teams of service providers to support each site within our portfolio. These teams visit sites on a regular basis to provide support to the tenants who live there and to boarding home operators and their staff.

Bailey House

 

Bailey House is a supportive housing program that provides rent-subsidized accommodation and support for 10 men living with mental health challenges and co-occurring diabetes or pre-diabetic conditions. Each tenant has his own bedroom and shares a bathroom and communal kitchen with other tenants.

Adams House

 

Adams House is a supportive housing program that provide rent-subsidized accommodation and support for 20 men and 6 women living with mental health challenges. Each tenant has his or her own bedroom and shares a bathroom and kitchenette with two or three other tenants.
