Dual Diagnosis and Developmental Disabilities Services

COTA Health provides services to individuals 16 years of age and older who have a developmental disability and dual diagnsosis (developmental disability and mental illness).

Community Liaison Worker (CLW) Program

 

COTA Health’s Community Liaison Worker (CLW) program provides case management services to young adults with developmental disabilities who were formerly in the care of a Toronto-based child welfare agency and who currently reside in a privately operated children’s residence.

Dual Diagnosis Case Management

 

COTA Health’s Dual Diagnosis Case Management service provides individualized support to clients living with a co-occurring developmental disability and a diagnosis of serious mental illness. In addition to this, the team also provides dedicated support to individuals with a dual diagnosis who also have justice system involvement (Dual Diagnosis & Justice Case Management – DD & J CM).
