Homelessness Services

COTA Health provides support services to individuals who are homeless or under-housed; and support individuals who have been recently housed to successfully maintain their housing.

ACT Team

 

COTA Health was selected, by the Mental Health Commission of Canada, to provide a Recovery Oriented Assertive Community Treatement (ACT) Team in Toronto. This is a four year project that started in 2009 and will operate until 2013 in Vancouver, Winnepeg, Montreal, Moncton and Toronto.

Hostel Outreach Program

 

The Hostel Outreach Program (HOP) provides individualized support to men who are homeless or under-housed and are living with a serious mental illness.
